Privacy policy

As part of using this tool, we will be collecting some of your personal information.

What we record in this tool

As a patient the following details will be recorded:

  • your name and health number (National Health Index or NHI)
  • confirmation of your consent to share your personal information
  • any other information you provide while using this tool

As a healthcare practitioner or Te Whatu Ora employee using this tool the following details will be recorded:

  • your name
  • your HPI CPN (Health Practitioner Index Common Person Number) when available
  • your unique My Health Account Workforce ID

Providing your information

By law, all health services are required to keep complete and accurate records. This means there is no ability to opt out of your information being recorded within this tool.

After your information is collected

The information collected by this tool will be used to provide you healthcare services and may be shared with healthcare practitioners involved in your care.

Information may also be used for statistical analysis and research purposes in line with usual Ministry of Health processes and when permitted by the Privacy Act 2020 and Health Information Privacy Code 2020.

If we need to use information collected by this tool for other purposes, we will have this reviewed by the Chief Privacy Officer at the Ministry of Health and discuss it with the Office of the Privacy Commissioner to make sure your privacy interests are fully considered.

Accessing and correcting your information

You can request a record of your personal information by:

  • Calling 0800 222 478 and pressing the relevant option or
  • your HPI CPN (Health Practitioner Index Common Person Number) when available

Who can access the information held by this tool

Only Health practitioners registered with the Health Practitioner Index and approved employees of Te Whatu Ora administration can see, add or change the information held by this tool. Access to this tool is limited to those with logon access rights, and all access is recorded and can be audited.

This may include your doctor, pharmacist, hospital staff, community healthcare provider, administration staff and other health professionals who work with you. Everyone who accesses this tool is trained to keep the information safe.

Storing your information

This tool is hosted on the HNZ FHIR server and is shared with other Te Whatu Ora systems as required. Access to the data remains limited to those with logon access rights, and all access is recorded and can be audited.

How long your information is kept

The information will be held in compliance with our standards. It will be kept for a minimum period of 30 days from collection, and longer if clinically determined to be appropriate.
